SUPPLEMENTAL/BID BULLETIN
Addendum No. 1
This Bid Bulletin is issued to inform prospective Bidders of some changes and/or modifications in the project requirements “Various Medicines for the use at Pharmacy Section for various patients of NOPH under Bid No. B-152-2013, ABC: P5,503,492.00”. This shall form an integral part of the Bid Documents.
- Items to be delivered 10 calendar days from receipt of Notice to Proceed.
- Additional Requirements for Post-qualification requirements [BDS Sec. 29.2(d)]
For items 48, 49, 96, 97, 98, 99, 100, 101, 102 ,103 and 104 – Suppliers must submit Certification of its Acceptability issued by End-user.
