Addendum No. 1

 

This Bid Bulletin is issued to inform prospective Bidders of some changes and/or modifications introduced by the Bids and Awards Committee for the following project requirements, viz:

 

Bid No. Project Title ABC:
B-233-2023 Purchase of One (1) Unit Fully Automated Microtomy Machine for the use at Laboratory Section of NOPH P2,000,000.00
B-234-2023 Purchase of One (1) Unit Fully Automated Embedding Machine for the use at Laboratory Section of NOPH P1,400,000.00
B-235-2023 Purchase of One (1) Unit Cautery Machine for the use at the Operating Section of NOPH P1,700,000.00

 

Additional Terms and Conditions:

 

  1. The Bidder/Supplier must provide Certificate of Exclusive Distributorship from the Manufacturer.

 

  1. The Bidder must be ISO 9001 and 13485 Certified.

 

 

Bidders are enjoined to take note and consider the above mentioned additional terms and conditions in the preparation of their bids.
